Abstract
El acceso convencional a la cavidad pulpar en dientes con calcificación del conducto radicular (CCR) es desafiante y suele estar asociado a perforaciones o falsas vías. El uso de guías endodónticas es un método prometedor en la resolución de estos casos. El objetivo del presente trabajo es el abordaje de una CCR en un incisivo central superior con falsa vía mediante guía endodóntica estática. Paciente masculino de 38 años de edad refiere cambio de coloración postraumatismo dentoalveolar en el diente 11. En radiografía periapical inicial se observó CCR parcial e imagen radiolúcida asociada al ápice. En un primer intento de localización del conducto radicular se realizó una apertura amplia y falsa vía, por lo que se indicó una tomografía computarizada de haz cónico (TCHC). Antes de la adquisición del volumen tomográfico, la falsa vía fue reparada y se decide realizar un nuevo acceso a través de una guía endodóntica estática. Se obtuvo un modelo digital mediante un escáner intrabucal, el mismo fue fusionado en el software de planificación digital con los archivos de la TCHC para diseñar la guía endodóntica, ésta fue impresa y colocada en el diente con la finalidad de guiar el acceso endodóntico, el cual fue realizado de forma expedita y precisa. Después de 18 meses, el paciente continua asintomático. Es importante destacar la importancia de la reparación de la falsa vía previa a la instrumentación guiada, lo que evitaría el desvió del fresado y otras complicaciones como la fractura del diente.
