Abstract
Pacjenci z nowotworami przewodu pokarmowego często doświadczają wyniszczenia organizmu, znanego jako kacheksja nowotworowa.To złożone zjawisko postępującego wyczerpywania tkanek organizmu objawia się uporczywą utratą masy ciała, której nie można odwrócić za pomocą konwencjonalnych metod wsparcia żywieniowego.Kacheksja nowotworowa znacząco obniża jakość życia pacjentów, zmniejsza możliwości leczenia choroby podstawowej i generuje obciążenia dla systemu opieki zdrowotnej.Kryteria diagnostyczne, obraz kliniczny i etapy tego zaburzenia są skomplikowane, a przebieg choroby zależy od indywidualnych cech pacjenta i choroby podstawowej.Mimo wielu badań skomplikowane mechanizmy powstawania kacheksji nowotworowej nadal nie są w pełni poznane, jednak wydają się związane z działaniem cytokin prozapalnych, czynników prokatabolicznych wydzielanych przez guz i zmian hormonalnych.Oprócz tych czynników istotną rolę odgrywają również zaburzenia apetytu, zmiany w metabolizmie białek i tłuszczów oraz mechaniczna blokada wchłaniania pokarmu w przewodzie pokarmowym, uwarunkowana umiejscowieniem nowotworu.Obecne postępowanie z pacjentami z kacheksją nowotworową ma charakter wielokierunkowy.Stosuje się leczenie niefarmakologiczne, w którym obecnie zalecane są interwencje multimodalne, łączące wiele strategii działających synergistycznie.Istnieją również możliwości leczenia farmakologicznego, jednak wiele z przebadanych substancji nie przyniosły oczekiwanych rezultatów.Celem niniejszej publikacji jest podsumowanie aktualnego stanu wiedzy na temat kacheksji w nowotworach przewodu pokarmowego, uwzględniając najnowsze doniesienia naukowe w tym obszarze.
