Abstract

Objetivo: Avaliar a capacidade pulmonar, funcional e muscular de pacientes com cirrose hepática (CH) quando comparado com indivíduos saudáveis. Métodos: Estudo observacional transversal realizado em pacientes com CH. A amostra foi de 40 indivíduos, sendo 22 pacientes com CH (Grupo Cirrose Hepática - GCH), 81,82% Child-Pugh B, sendo a maioria do sexo masculino (n=14) com idade média de 59,86±8,13 e 18 indivíduos controles (Grupo Controle - GC) pareados com o GCH. Foram registrados os dados da prova de função pulmonar por meio de espirometria, força muscular periférica isométrica de extensão da articulação do joelho por um dinamômetro isocinético poliarticular, capacidade de exercício pelo Teste da Caminhada de 6 Minutos (TC6) e oxigenação muscular periférica por meio da espectroscopia no infravermelho próximo. Resultados: Os valores do GCH foram menores quanto ao volume expiratório forçado no primeiro segundo quando comparada ao predito (80,69±14,67 vs. 95,59±12,20% do predito, p<0,01), força muscular periférica direita (117,55±44,00 vs.148,86±47,54Nm, p=0,02), distância percorrida no teste de caminhada de seis minutos (TC6) (416,91±111,65 vs. 576,82±72,22m, p<0,01) e índice de saturação tecidual (IST) antes e após o TC6 respectivamente (64,27±12,31 vs.76,36±8,02%, p<0,01) e (65,08±14,46 vs. 78,46±9,05%, p<0,01) quando comparado ao GC. Houve correlação moderada no GCH, entre a distância percorrida no TC6 e a força muscular periférica direita (rho=0,46; p=0,03) e esquerda (rho=0,53; p=0,01). Conclusão: Os pacientes com CH apresentam alterações na capacidade de exercício, no IST e na força muscular periférica e esta redução interfere na capacidade de exercício dessa população.
