Abstract

Nghiên cứu nhằm đánh giá kết quả điều trị sẹo lồi bằng tiêm Triamcinolone acetonide (TAC) kết hợp Botulinum toxin A (BTA) trong tổn thương. Nghiên cứu thử nghiệm lâm sàng không đối chứng trên 26 bệnh nhân với 49 sẹo lồi. Trước điều trị trung bình độ dày sẹo là 1,86 ± 0,76, độ cứng là 3 ± 0,84 và tổng điểm VSS (Vancouver Scar Score sheet) là 7,53 ± 2,22. Sau điều trị các giá trị tương ứng giảm lần lượt là 0,43 ± 0,65; 0,53 ± 0,65 và 4,29 ± 2,82 (p < 0,05). Điểm trung bình trước điều trị của sắc tố sẹo là 3 ± 0,84 và mạch máu sẹo là 2,02 ± 0,80, sau điều trị tăng lần lượt là 1,02 ± 1,01 và 2,33 ± 0,94 (p < 0,05). Điểm trung bình POSAS (Patient and Observer Scar Assessment Scale) của người quan sát trước điều trị là 31,02 ± 7,40, sau điều tri là 14,29 ± 4,96 (p < 0,05) và của bệnh nhân tự đánh giá tương ứng là 37,45 ± 9,59 và 14,16 ± 4,75 (p < 0,05). Trung bình tỷ lệ cải thiện độ dày sẹo sau điều trị là 79,93% ± 29,65%, trong đó 63,3% sẹo xẹp hoàn toàn. Nghiên cứu của chúng tôi cho thấy điều trị sẹo lồi bằng tiêm kết hợp TAC và BTA có hiệu quả cao: giảm rõ rệt triệu chứng cơ năng và độ dày sẹo, đồng thời có tỷ lệ hài lòng cao của bệnh nhân.
