Durée d’hypoglycémie similaire avec icodec vs degludec ou glargine U100 chez les patients DT2 insulinotraités : analyse CGM post-hoc d’ONWARDS 2 et 4

Abstract
icodec est une insuline basale hebdomaire qui a été étudiée dans le cadre du programme ONWARDS. Le temps passé en hypoglycémie pendant les périodes de switch (S0-4) et d’état d’équilibre (S22-26) de deux essais de phase 3 sur des patients DT2 sous insuline a été étudié. Les patients ont été randomisés entre icodec ou degludec quotidienne (ONWARDS 2, n = 526), ou icodec ou glargine U100 quotidienne en schéma basal-bolus avec insuline asparte prandiale (ONWARDS 4, n = 582). Le temps médian des hypoglycémies < 3,9 mmol/L pour les épisodes d’une durée ≥ 15 minutes et la proportion de ces épisodes passés < 3,0 mmol/L ont été évalués. Le temps médian passé en hypoglycémie < 3,9 mmol/L était similaire entre icodec et les comparateurs pendant le switch (icodec/degludec et icodec/glargine U100, 40/35 et 40/40 minutes) et à l’état d’équilibre (35/35 et 40/40 minutes), sans différenciation entre les deux périodes. À l’état d’équilibre (icodec/degludec et icodec/glargine U100), parmi les épisodes < 3,9 mmol/L, 68,2/63,5 % et 60,9/60,6 % n’ont pas atteint le seuil < 3,0 mmol/L ; 21,8/26,4 % et 28,1/28,2 % ont présenté des épisodes < 3,0 mmol/L pendant ≥ 15 minutes ; et 10,0/10,1 % et 11,0/11,3 % ont présenté des épisodes < 3,0 mmol/L pendant < 15 minutes, respectivement. chez ces patients DT2 insulinotraités, le temps passé en hypoglycémie < 3,9 mmol/L était similaire pour icodec vs degludec ou glargine U100. Ces résultats montrent qu’avec l’insuline icodec qui a une demi-vie d’environ 1 semaine, la durée de l’hypoglycémie n’est pas prolongée vs degludec ou glargine U100.
