CGM chez des patients DT2 insulinotraités switchés pour l’insuline icodec hebdomadaire vs comparateurs journaliers : post-hoc des études ONWARDS 2 et 4

Abstract
Le temps passé dans, au-dessus et en dessous de la cible (TIR, TAR, TBR) de deux essais de phase 3 sur des patients DT2 sous insuline, randomisés à l’insuline hebdomadaire icodec a été étudié. Des patients DT2 insulinotraitées ont reçu icodec ou dégludec journalière (ONWARDS2), ou icodec ou glargine U100 journalière avec de l’insuline asparte au moment des repas (ONWARDS4). Le TIR (3,9–10,0 mmol/L), le TAR (> 10,0 mmol/L) et le TBR (< 3,9 et < 3,0 mmol/L) ont été calculés. Post-switch, le TIR, le TAR et le TBR n’étaient pas significativement différents entre icodec et les comparateurs. À l’équilibre, il n’y avait pas de différence significative pour le TIR ou le TAR. Dans ONWARDS2, le TBR < 3,9 mmol/L était significativement plus long avec icodec (RR estimé : 1,59 [IC95 % : 1,21 ; 2,08] ; p = 0,001). La proportion de participants d’ONWARDS2 ayant atteint les trois objectifs CGM (> 70 % TIR, < 25 % TAR et < 4 % TBR < 3,9 mmol/L) à l’état d’équilibre était de 28,2 % et 22,2 % avec icodec et degludec, respectivement (RR estimé : 1,23 [IC95 % : 0,80 ; 1,88] ; p = 0,348). Dans ONWARDS4, 29,1 % (icodec) et 31,7 % (glargine U100) des patients ont atteint ces objectifs (RR estimé : 0,91 [IC95 % : 0,62 ; 1,34] ; p = 0,643). Entre le switch et l’état d’équilibre, le TIR moyen global observé a augmenté, le TAR a diminué et le TBR est resté en dessous des objectifs recommandés dans tous les groupes. Chez les patients DT2 insulinotraitée, le TIR et le TAR n’étaient pas significativement différents vs degludec ou glargine U100.
