Pénétrance de l’hyperplasie macronodulaire bilatérale des surrénales GIP-dépendante avec syndrome de Cushing chez les porteurs de mutations de KDM1A

Abstract
L’hyperplasie macronodulaire bilatérale des surrénales (HMBS) GIP-dépendante est une maladie génétique liée à une inactivation du gène KDM1A selon un modèle de gène suppresseur de tumeur. Notre objectif était d’évaluer la pénétrance et l’histoire naturelle de l’atteinte surrénalienne chez des porteurs de variants pathogènes de KDM1A. Nous avons réalisé un dépistage des apparentés de 17 patients atteints d’HMBS GIP-dépendante comportant le séquençage de KDM1A puis un dosage du cortisol plasmatique à jeun et post-prandial, un cortisol libre urinaire des 24 h, une électrophorèse des protéines sériques, et un scanner surrénalien chez les sujets porteurs. Nous avons étudié 13 familles : 22 sur 37 apparentés genotypés étaient porteurs d’un variant pathogène de KDM1A . Nous avons identifié une nouvelle patiente avec HMBS GIP-dépendante et hypercortisolisme évident mais non diagnostiqué. Les autres porteurs de variant de KDM1A ne présentaient pas d’anomalies de la sécrétion de cortisol : cortisol plasmatique 12,4 ± 4,9 μg/dL à jeun et 10 ± 4,5 μg/dL en post-prandial, cortisol libre urinaire 33 ± 12,7 μg/24 h ; deux apparentés avaient une gammapathie monoclonale de signification indéterminée. Le volume surrénalien était normal (surrénale droite = 5,4 ± 1,9 cm 3 et gauche = 5,8 ± 3,3 cm 3 ) et elles étaient sans nodules. Parmi les 22 apparentés porteurs d’une mutation de KDM1A , nous avons retrouvé un nouveau cas d’HBMS GIP-dépendante. Nous poursuivons le dépistage chez un plus grand nombre d’individus afin de pouvoir estimer précisément la pénétrance de la pathologie surrénalienne et son évolution chez des porteurs de variants pathogènes de KDM1A.
