Введение. При псориазе и атопическом дерматите часто встречаются сопутствующие психические расстройства. Цель. Изучить вклад алекситимии, копинг-стратегий, механизмов психологических защит и ряда клинических показателей в развитие расстройства адаптации при псориазе и атопическом дерматите. Выявить предикторы неблагоприятного течения псориаза и атопического дерматита. Материалы и методы. Обследовано 67 пациентов с псориазом, 59 – с атопическим дерматитом. Интенсивность зуда изучалась визуально-аналоговой шкалой. Алекситимия оценивалась 20-пунктовой Торонтской шкалой алекситимии. Стрессоустойчивость определялась методом Холмса – Раге. Копинг-стратегии трактовались методом «Способы совладающего поведения» по Лазарусу. Психологические защиты изучались методом Плутчика – Келлермана – Конте. Соматизация оценивалась опросником SOMS-2. Результаты. При псориазе расстройство адаптации ассоциировано с алекситимией (OR=4,81 [CI=1,43–16,2]; p=0,008), трудностью описания чувств (OR=6,17 [CI=1,49– 25,6]; p=0,01), внешне-ориентированным мышлением (OR=3 [CI=1,08–8,32]; p=0,032); вытеснением (OR=3,23 [CI=1,14–9,14]; p=0,025), конфронтацией (OR=3,25 [CI=1,02– 10,3]; p=0,04), дистанцированием (OR=2,83 [CI=1,03–7,78]; p=0,041) и самоконтролем (OR=3,5 [CI=1,02–12]; p=0,04); бегством-избеганием проблемы (OR=3,23 [CI=1,14– 9,14]; p=0,025). При атопическом дерматите расстройство адаптации ассоциировано с трудностью идентификации чувств (OR=3,14 [CI=1,03–9,55]; p=0,04); проекцией (OR=3,66 [CI=1,02–13,2]; p=0,041) и соматизацией (OR=4,26 [CI=1,21–15]; p=0,019); большим количеством госпитализаций (OR=3,65 [CI=1,15–11,4]; p=0,024) и длительной последней ремиссией (OR=3,23 [CI=1,08–9,71]; p=0,034). Предикторы неблагоприятного течения псориаза: стрессоустойчивость (OR=1 [CI=1–1,01]; p=0,01), гиперкомпенсация (OR=1,09 [CI=1,03–1,16]; p=0,005) и интенсивность зуда (OR=1,78 [CI=1,3–2,45]; p<0,001). Предикторы неблагоприятного течения атопического дерматита: трудность описания чувств (OR=1,22 [CI=1,03–1,46]; p=0,021) и общая напряженность механизмов психологических защит (OR=1,11 [CI=1,04–1,18]; p=0,002). Заключение. Коррекция эмоциональной осведомленности, обучение адаптивным копинг-стратегиям и зрелым механизмам психологических защит – мишени консультативно-психиатрической помощи пациентам с псориазом и атопическим дерматитом при наличии расстройства адаптации. Introduction. In psoriasis and atopic dermatitis psychiatric comorbidities are common. Purpose. To assess link between alexithymia, psychological defense mechanisms, coping strategies, some clinical parameters and presence of the adjustment disorder in patients with psoriasis and atopic dermatitis. To identify unfavorable course predictors of psoriasis and atopic dermatitis. Materials and methods. n=67 patients with psoriasis and n=59 patients with atopic dermatitis were examined. The intensity of itching was assessed by Visual Analogue Scale. Alexithymia was assessed by Toronto alexithymia scale. Stress level was assessed by The Holmes and Rahe stress-scale. Coping strategies were assessed by Ways of coping questionnaire. Psychological defense mechanisms were assessed by The Life Style Index. Somatization was assessed by SOMS-2. Results. In psoriasis presence of adjustment disorder was associated with alexithymia (OR=4,81 [CI=1,43–16,2]; p=0,008), difficulty describing feelings (OR=6,17 [CI=1,49–25,6]; p=0,01), externally oriented thinking (OR=3 [CI=1,08–8,32]; p=0,032); repression (OR=3,23 [CI=1,14–9,14]; p=0,025); confrontational coping (OR=3,25 [CI=1,02–10,3]; p=0,04), distance coping (OR=2,83 [CI=1,03–7,78]; p=0,041), self-control coping (OR=3,5 [CI=1,02– 12]; p=0,04) and escape-avoidance coping (OR=3,23 [CI=1,14–9,14]; p=0,025). In atopic dermatitis presence of adjustment disorder was associated with difficulty identifying feelings (OR=3,14 [CI=1,03–9,55]; p=0,04); projection defense mechanism (OR=3,66 [CI=1,02–13,2]; p=0,041), somatization defense mechanism (OR=4,26 [CI=1,21–15]; p=0,019); large number of hospitalizations (OR=3,65 [CI=1,15–11,4]; p=0,024) и long last remission (OR=3,23 [CI=1,08–9,71]; p=0,034). Predictors of psoriasis unfavorable course: level of stress resistance (OR=1 [CI=1–1,01]; p=0,01); hypercompensation (OR=1,09 [CI=1,03–1,16]; p=0,005); itching intensity (OR=1,78 [CI=1,3–2,45]; p<0,001)). Predictors of atopic dermatitis unfavorable course: difficulty describing feelings (OR=1,22 [CI=1,03–1,46]; p=0,021); intensity of psychological defense mechanisms (OR=1,11 [CI=1,04–1,18]; p=0,002)). Conclusion. The main targets for consultation-liaison psychiatry in patients with psoria- sis and atopic dermatitis with adjustment disorder: emotional awareness improvement, adaptive coping strategies and mature psychological defense mechanisms training.
