Assessment of the components of clinical remission with moderate–severe asthma after 24–52 weeks of treatment: CAPTAIN post hoc analysis

Abstract
Objective: Clinical Remission (CR) is an emerging goal for patients with asthma assessed after 52 weeks of therapy. This post hoc analysis of CAPTAIN assessed CR at Week 52 (w52) in patients with moderate–severe asthma receiving FF/UMEC/VI or FF/VI who did/did not meet components of CR at Week 24 (w24). Methods: CAPTAIN compared FF/UMEC/VI vs FF/VI in adults with moderate–severe asthma uncontrolled despite ICS/LABA; history of severe exacerbation was not required in the prior year. CR was defined as no SCS use, no severe exacerbations, ACQ-5 ≤1.5, optimised lung function. Outcome: proportion of patients receiving FF/UMEC/VI (100/62.5/25, 200/62.5/25µg) and FF/VI (100/25, 200/25µg) who achieved CR at w52, according to if CR components were met at w24. Results: Of 91/408 patients receiving FF/UMEC/VI 200/62.5/25 who continued to w52, 34/91 (37%) met the CR components at w24; of whom 23/34 (68%) achieved CR at w52. Of the 57/91 (63%) who did not meet the CR components at w24, 12/57 (21%) achieved CR by w52 (Table). Conclusion: Most patients who met the CR components at w24 achieved CR at w52, indicating w24 response is related to w52 CR. A limitation is the low number of subjects with w52 data. Prospective research is needed to establish practical assessment timepoints, durability of early results, and long-term utility of CR as a treatment goal in asthma.
