Цель исследования – установить специфические особенности показателей состояния сосудистой стенки у пациентов с субъективным когнитивным снижением (СКС). Материал и методы. Обследованы 88 условно здоровых мужчин 50–52 лет. Применены нейропсихологические методики исследования когнитивных функций: Монреальская шкала оценки когнитивных функций, батарея тестов лобной дисфункции, тест вербальных ассоциаций (литеральных и категориальных), тесты запоминания 12 слов, рисования часов, зрительной памяти (Бентон), прокладывания маршрута (A и B), символьно-цифрового соответствия. Эмоционально-аффективную сферу оценивали с помощью опросников депрессии Бека и тревоги Спилбергера–Ханина. Состояние артериальной стенки определяли посредством аппланационной тонометрии, измерения поток-зависимой вазодилатации плечевой артерии, лазерной допплеровской флоуметрии, фотоплетизмографии с дилататорными и констрикторными пробами. Для установления наличия/отсутствия изменений в веществе головного мозга всем пациентам проведена магнитно-резонансная томография. Результаты. Выделены две группы пациентов: без когнитивных нарушений (бКН) и с СКС. У пациентов бКН отмечены более высокие показатели жесткости артериальной стенки (центральное систолическое аортальное давление и центральное диастолическое аортальное давление). У пациентов с СКС был выше уровень сатурации (SpO2), а также более выраженные изменения по шкалам депрессии Бека и тревоги Спилбергера–Ханина. Заключение. Полученные инструментальные данные свидетельствуют о вероятном нарушении адаптивных свойств артериальной стенки у пациентов среднего возраста с СКС. Objective. To establish specific features of vascular wall indicators in patients with subjective cognitive decline (SCD). Material and methods. Eighty-eight healthy men aged 50–52 years were examined. Neuropsychological methods were used to study cognitive impairment: Montreal Cognitive Assessment, Frontal Assessment Battery, verbal associations (literal and categorical) method; "12 Words" test, Clock Drawing Test, Benton Visual Retention Test, Trail Making Test A and B, Symbol-Digit Modalities Test. The emotional-affective background was assessed using the Beck Depression Inventory (BDI) and the Spielberger– Hanin Reactive and Personality Anxiety Scale (SPRA). Instrumental methods were used to study the arterial wall: applanation tonometry, brachial artery flow-mediated dilatation, Celermajer index, laser Doppler flowmetry, photoplethysmography. All patients have been completed magnetic resonance imaging of the brain. Results. Two groups of individuals were identified: cognitive healthy (CH) and SCD. CH patients differed in certain parameters by higher arterial wall stiffness: central systolic aortic pressure and central diastolic aortic pressure. Patients with SCD showed higher levels of oxygen saturation (SpO2) and more pronounced changes in BDI, SRPA. Conclusion. The obtained instrumental data probably indicate impaired adaptive properties of the arterial wall in middle-aged patients with SCD.
