Uso de oclacitinib no tratamento de pênfigo foliáceo canino: relato de caso

Abstract
O pênfigo foliáceo é a doença cutânea autoimune frequente na rotina clínica em cães e gatos. Trata-se de uma dermatose pustular ocasionada pelo rompimento das aderências desmossômicas dos queratinócitos. O pênfigo se apresenta como uma lesão cutânea primária, denominada pústula subcórnea. O diagnóstico é firmado por meio da associação do quadro clínico, seguido da confirmação histopatológica, identificando-se a presença de acantólise subcorneal. Foi realizado o atendimento especializado em dermatologia veterinária de um cão fêmea, da raça labrador, de 10 anos de idade, que apresentava prurido intenso há 11 meses sem antecedentes. O exame físico evidenciou a presença de eritema em ambos os condutos auditivos e havia predomínio de lesões crostosas e pustulares, com localização bilateral e simétrica em região torácica, membros torácicos e pélvicos, pavilhões auriculares, região periocular e ponte nasal. Foi realizada a biópsia das lesões, confirmando o diagnóstico de pênfigo foliáceo. Como forma de tratamento, foi instituído o uso de oclacitinib em dose, inicialmente, elevada (1mg/kg, BID), sendo este um inibidor da Janus quinase (JAK), aprovado para tratar dermatite atópica (DA) em cães, que age como um inibidor da função de importantes citocinas pró-inflamatórias e pruridogênicas por meio da inibição do transdutor de sinal Janus quinase (JAK) e da via de sinalização do ativador da transcrição (STAT). A paciente apresentou melhora clínica significativa, sem efeitos adversos, e seus exames laboratoriais permaneceram sem alterações durante todo o tratamento.
