Abstract
Este relato descreve o caso de um homem de 28 anos, HIV positivo desde 2022, em terapia antirretroviral com Tenofovir, Lamivudina e Dolutegravir. O paciente apresentava febre persistente, anorexia, icterícia progressiva, fadiga acentuada e lesões ulceradas perianais. Ao exame físico, constatou-se hepatoesplenomegalia, linfonodomegalias cervicais, axilares e inguinais, além de ascite leve e edema periférico. Os exames de imagem revelaram linfonodomegalias retroperitoneais e inguinais, hepatoesplenomegalia e líquido livre na pelve. A biópsia linfonodal confirmou hiperplasia linfóide com padrão hialino-vascular, característico de DCM. O PCR para EBV foi positivo, reforçando o papel do vírus na patogênese da condição. O manejo incluiu rituximabe, um anticorpo monoclonal anti-CD20, administrado semanalmente por quatro semanas, visando controlar a proliferação linfocitária mediada por células B. A prednisona, em dose de 1 mg/kg/dia, foi adicionada para reduzir a inflamação sistêmica. O paciente também recebeu transfusões de hemácias e plaquetas devido à pancitopenia severa. A TARV foi suspensa temporariamente devido à toxicidade hepática, com planejamento de reintrodução após estabilização clínica. A resposta ao tratamento foi satisfatória, com melhora dos sintomas sistêmicos e controle da linfoproliferação. O caso ilustra a complexidade do diagnóstico e manejo da DCM em pacientes imunossuprimidos. A combinação de hepatoesplenomegalia, linfonodomegalias e ascite leve, associada à hiperplasia linfóide típica, foi essencial para o diagnóstico. Coinfecções virais, como o EBV, são frequentes em pacientes com DCM associada ao HIV, agravando o quadro clínico e complicando o manejo.
