Analysis of CT characteristics and prognostic factors in non-hypertensive cerebral hemorrhage

Abstract

To analyze the CT features and prognostic factors of non-hypertensive cerebral hemorrhage. Sixty-seven hypertensive cerebral hemorrhage patients were included in Group A, and forty non-hypertensive cerebral hemorrhage patients were assigned to Group B. Additionally, 60 healthy individuals who underwent physical examinations during the same period were included in Group C. The basic data, laboratory parameters, Glasgow Outcome Scale (GOS), Neurological Deficit Scale (NDS), Montreal Cognitive Assessment (MoCA), and CT findings were compared among the three groups. The levels of fasting blood glucose (FBG), glycated hemoglobin (HbA1c), triglycerides (TG), low-density lipoprotein (LDL), intercellular adhesion molecule-1 (ICAM-1), and fibrinogen (FIB) in Group B were found to be significantly higher compared to both Group A and Group C. In contrast, the levels of white blood cell count (WBC), neutrophils (N), neutrophil-to-platelet ratio (NPR), red blood cell distribution width (RDW), RDW coefficient of variation (RDW-CV%), RDW standard deviation (RDW-SD), C-reactive protein (CRP), interleukin-6 (IL-6), interleukin-17 (IL-17), matrix metalloproteinase-1 (MMP-1), tumor necrosis factor-α (TNF-α), and interferon-γ (IFN-γ) were significantly higher in both Groups A and B when compared to Group C. Moreover, matrix metalloproteinase-9 (MMP-9), brain natriuretic peptide (BNP), D-dimer, serum amyloid A (SAA), neuron-specific enolase (NSE), and S100 protein levels in Group B were significantly lower than those in Group A, but notably higher than those in Group C. All these differences were statistically significant (P < 0.05). Group A exhibited significantly greater blood loss, hematoma volume, cerebral blood flow (CBF), cerebral blood volume (CBV), and mean transit time (MTT) compared to Group B, with these differences being highly significant (P < 0.001). Additionally, multivariate logistic regression analysis identified several independent risk factors for the prognosis of non-hypertensive cerebral hemorrhage. These included diabetes, hyperlipidemia, obesity, arteriovenous malformation (AVM), cerebral aneurysm, cerebral hemorrhage of unknown origin, as well as specific biomarkers such as HbA1c, TG, LDL, HDL, FIB, S100, hematoma volume, CBV, and MTT. All of these factors were found to be statistically significant (P < 0.05). The occurrence of non-hypertensive cerebral hemorrhage is primarily associated with factors such as diabetes, hyperlipidemia, obesity, and arteriovenous malformation (AVM). Compared to hypertensive cerebral hemorrhage, the CT characteristics of non-hypertensive cerebral hemorrhage mainly include a smaller volume of hemorrhage and lower cerebral blood flow.
