Abstract
vod: Ndory ledviny T3b, tedy s ndorovm trombem v renln le i doln dut le (DD), jsou standardn povaovny za kontraindikaci k laparoskopii. V prci se sname podat nov poznatky o tto problematice. Vlastn soubor: V obdob 1/2003 a 6/2005 jsme provedli 71 radiklnch laparoskopickch i retroperitoneoskopickch nefrektomi pro parenchymov tumor. U 16 (22,5 %) se jednalo o kategorii pT3b, tedy histopatologicky prokzan prorstn tumoru do vtv renlnch il. U 15 z nich vak byl tumor klinicky kategorie cT1 i cT2, teprve pi histopatologickm vyeten se prokzalo prorstn do vtv renlnch il (pT3b). Hlavn kmen renln ly byl tedy vdy voln, take technika operace nebyla odlin od kategori pT1 i pT2. Pouze u jednoho 57letho mue s tumorem prav ledviny se jednalo o trombus v renln le verifikovan ji pedoperan. Rozsah tumoru byl uren dle multidetektorovho CT vetn CT-angiografie - trombus konil 1 cm od doln dut ly. Byla provedena laparoskopie, peruen renln ly pomoc endoGIA stapleru tsn u DD. Nsledn klipy peruena arterie pstupn a po peruen ly. Ledvina i s nadledvinou odstranny en bloc. Odstranna i uzlina nalhajc na renln lu a DD. as operace byl 170 min, krevn ztrta 200 ml. Histologicky se jednalo o svtlobunn renln karcinom pT3b pN0 G3. Pacient zhojen bez komplikac.Zvr: Ve vybranch ppadech lze laparoskopicky eit i tumory s ndorovm trombem v renln le. Rozhodnut o typu vkonu je mon jen na zklad multidetektorovho CT, vetn CT-angiografie i pomoc vysoce vkonnho MRI.
