Thrombospondin-4 als potentieller Liquor-Biomarker für die pädiatrische 5q-assoziierte spinale Muskelatrophie

Abstract
Die Spinale Muskelatrophie (SMA) wird durch einen Mangel an SMN-Protein verursacht, der zur Degeneration von Motoneuronen (MN) und Muskelschwäche führt. Der Verlust von MN kann jedoch nicht nur auf den SMN-Mangel zurückzuführen sein. Retrograde Signale aus Skelettmuskeln und neuromuskulären Verbindungen (NMJs) spielen wahrscheinlich eine Rolle bei der MN-Schädigung. Momentan gibt es keine SMA-Biomarker, die mit der neuromuskulären Übertragung in Verbindung stehen. Diese könnten Auskunft über die Rolle der NMJs bei der SMA liefern. Daher untersuchten wir in dieser Studie Thrombospondin-4 (TSP4), welches an der NMJ lokalisiert ist, als potenziellen Liquor(CSF)-Biomarker. Mittels Proteomanalysen wurden Biomarker in CSF-Proben von symptomatischen und präsymptomatischen pädiatrischen SMA-Patienten bestimmt. Der identifizierte Kandidat TSP4 wurde dann in CSF-Proben von weiteren pädiatrischen und erwachsenen SMA-Patienten sowie von nicht erkrankten und erkrankten Kontrollen mittels Enzymimmunoassay (ELISA) quantifiziert. Die Proteomik zeigte eine Dysregulation von TSP4 zwischen präsymptomatischen und symptomatischen Patienten. Weitere ELISA-Analysen wiesen einen TSP4-Abfall nur bei pädiatrischen, nicht aber bei erwachsenen SMA-Patienten oder pädiatrischen Patienten mit anderen neurologischen Erkrankungen nach. Zudem stieg der TSP4-Spiegel im CSF nach Verabreichung von Nusinersen bei pädiatrischen SMA-Patienten wieder an, was ein Potenzial als Therapie-Marker indiziert.
