Zyklische und kontinuierliche Dosierung von intravenösem Efgartigimod bei generalisierter Myasthenia gravis: Studiendesign ADAPT NXT

Abstract
Einführung: In der Studie der Phase 3b ADAPT NXT (NCT04980495) werden die Wirksamkeit, Sicherheit und Verträglichkeit von 10 mg/kg Efgartigimod i. v. in einem festen Zyklus (FZ; 4 Infusionen einmal wöchentlich mit einer 4-wöchigen Pause zwischen den Zyklen) bzw. kontinuierlichem Dosierungsschema (KD; alle 2 Wochen) untersucht. Methodik: Acetylcholinrezeptor-Antikörper-positive Erwachsene mit gMG (n=69), erhalten 4 Infusionen Efgartigimod in der 3-wöchigen Induktionsphase vor der Randomisierung (31) mit zyklischem oder kontinuierlichem Dosierungsschema in der 21-wöchigen Vergleichsphase. Nach dem letzten Zyklus erhalten die FZ-Teilnehmer Efgartigimod gemäß kontinuierlichem Dosierungsschema. Die Teilnehmer werden über einen Verlängerungszeitraum von 105 Wochen beobachtet, wobei diejenigen, bei denen die klinischen Verbesserungen weiterhin bestehen, für eine optionale Dosisminderung (alle 3 Wochen) infrage kommen. Ergebnisse: Der primäre Endpunkt ist definiert als die mittlere Veränderung des MG-ADL (Myasthenia gravis – Activities of Daily Living, Aktivitäten des täglichen Lebens) Scores insgesamt von Baseline bis Woche 21. Es werden Sicherheit, Verträglichkeit und pharmakologische / pharmakodynamische Wirkungen untersucht. Es werden die Ergebnisse aus der o.g. Analyse präsentiert. Schlussfolgerung: Die Studie ADAPT NXT wird wichtige Daten zu unterschiedlichen Dosierungsschemata liefern und so eine Individualisierung der Behandlung mit Efgartigimod bei Patienten mit gMG ermöglichen.
