Dominante SPTAN1 missense Variante assoziiert mit kongenitalen Malformationen und Myopathie

Abstract
Pathogene SPTAN1-Varianten führen zu sogenannten Spectrinopathien, die von Erkrankungen des peripheren oder zentralen Nervensystems bis zu komplexen syndromalen Phänotypen reichen. Heterozygote SPTAN1 Frameshift-Varianten wurden kürzlich mit der Manifestation einer distalen Myopathie mit neurogenen Symptomen in Verbindung gebracht. Durch Exom-Sequenzierung identifizierten wir eine heterozygote Missense-Variante (c.6923G>A; p.Arg2308His) in SPTAN1 bei einem Patienten mit kongenitaler Tetradysmelie, Oligosyndaktylie der Hände und Füße, fehlenden Acetabulardächern der Hüften, Klumpfuß (links) und Myopathie. Mit 2 Jahren lernte er mit Hilfe von Orthesen zu laufen, mit 12 Jahren wurde ein zunehmender Verlust der proximalen Muskelkraft festgestellt und mit 16 Jahren verlor er die Gehfähigkeit. Zeitgleich entwickelte er eine ausgeprägte Skoliose und eine zunehmende respiratorische Insuffizienz mit Beginn der BIPAP-Beatmung in der Nacht und der Mundstückbeatmung seit dem Alter von 20 Jahren. Die Muskelbiopsie, im Alter von 16 Jahren, ergab eine dystrophes Gewebsbild mit ausgeprägten Faserkaliberschwankungen, zahlreichen zentralen Kernen und schwerer peri- und endomysialer Fibrose. Elektronenmikroskopische Untersuchungen ergaben eine Veränderung der endothelialen Basalschicht und der Gefäße. Der schwerere Phänotyp unseres Patienten könnte durch einen anderen molekularen Mechanismus (basierend auf einer Missense-Variante) verursacht werden als die Haploinsuffizienz.
