Serglycin und Periostin als potenzielle neue Serum-Biomarker für die GNE-Myopathie

Abstract
Die GNE-Myopathie (GNEM) ist eine rezessive distale Myopathie, die durch eine im frühen Erwachsenenalter einsetzende, moderat fortschreitende distale Muskelschwäche gekennzeichnet ist. Sie wird durch bi-allelische Varianten im GNE -Gen verursacht, das für das bifunktionale Enzym UDP-N-Acetylglucosamin-2-Epimerase/N-Acetylmannosamin-Kinase kodiert, welches im Golgi-Apparat lokalisiert ist. Muskelbiopsien zeigen das Vorhandensein von umrandeten Vakuolen und gelegentlich Fibrose in einigen Bereichen. Außer Myostatin gibt es keine minimal-invasiven Biomarker für GNEM. Zur Identifikation minimal-invasiver Biomarker direkter pathophysiologischer Relevanz wurden in dieser Studie proteomische Analysen an Serumproben von GNEM-Patienten (n=15) durchgeführt. Die dabei identifizierten Biomarker-Kandidaten, Serglycin und Periostin, wurden anschließend mittels Enzymimmunoassay (ELISA; n=25) quantifiziert. Die proteomischen Befunde zeigten eine Reduktion von Serglycin und Periostin im Patienten-Serum, was durch anschließend durchgeführte ELISA-Messungen bestätigt werden konnte. Die pathophysiologische Relevanz von verändertem Periostin als Fibrosemarker wurde durch Immunfärbung an Muskelbiopsien gezeigt. Serglycin könnte als Golgi-Protein ein direktes Target des GNE-Proteins darstellen. Die Ergebnisse unserer kombinierten Analysen ließen dementsprechend die Identifikation von Periostin und Serglycin als zwei neue minimal-invasive Biomarker pathophysiologischer Relevanz für GNEM zu Publication History Article published online: 11 March 2025 © 2025.
