Die p.(Leu97Ile)-Variante erweitert das genetische Spektrum der NEFL-assoziierten Charcot-Marie-Tooth-Neuropathien

Abstract
Einleitung: Die Charcot-Marie-Tooth-Krankheit (CMT) beschreibt eine heterogene Gruppe hereditärer Neuropathien, die durch distale Muskelschwäche, Atrophien und sensorische Defizite gekennzeichnet ist. Mutationen im NEFL -Gen, das für das neurofilamentäre Leichtkettenprotein kodiert, sind mit bestimmten CMT-Subtypen assoziiert. In dieser Studie wird erstmals die p.(Leu97Ile)-Variante im NEFL -Gen beschrieben, die eine neue genetische Facette der NEFL -assoziierten Neuropathien darstellt. Methoden: Zwei Patienten aus nicht-verwandten Familien wurden klinisch untersucht. Die Symptomlast wurde mittels CMTESv2 und CMT-FOM erfasst. Genetische Analysen wurden mittels Exom-Sequenzierung durchgeführt. Hautbiopsien zur Bestimmung der Nervenfaserdichte und Blutproben für ELISA-Analysen von Neuropathie-Biomarkern wurden entnommen, die derzeit ausgewertet werden. Resultate: Beide Patienten zeigen typische CMT-Symptome, darunter sensomotorische Paresen und Atrophien. Elektrophysiologisch ergab sich eine sensorisch führende, axonal-demyelinisierende Polyneuropathie. Die genetische Analyse identifizierte bei beiden die heterozygote p.(Leu97Ile)-Variante im NEFL -Gen. Bei einem dieser Patienten wurde zusätzlich die p.(Arg206Ser)-Variante im NEFL -Gen nachgewiesen. Schlussfolgerung: Die Identifizierung der p.(Leu97Ile)-Variante liefert neue Einblicke in das genetische Spektrum der NEFL -assoziierten Neuropathien.
