Abstract
วิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาเพื่อสร้างระบบฐานข้อมูลมาตรฐานที่มีประสิทธิภาพสามารถรองรับระบบวิจัยในอนาคตในตัวอย่างทั้ง 3 กลุ่มและเพื่อหาค่าความสัมพันธ์ของแบบทดสอบทางประสาทจิตวิทยาโดยคอมพิวเตอร์ในศูนย์ผู้ดูแลผู้ป่วยสมองเสื่อม กลุ่มตัวอย่างที่ใช้ในการวิจัยครั้งนี้เป็นกลุ่มผู้สูงอายุที่มีอายุ ตั้งแต่ 55 ปีขึ้นไปที่มารับการตรวจที่แผนกผู้ป่วยนอกโรงพยาบาลจุฬาลงกรณ์ โดยแบ่งกลุ่มตัวอย่างเป็น 3 กลุ่มการทดลอง ได้แก่ กลุ่มผู้สูงอายุที่มีภาวการณ์รู้คิดปกติ (Normal), กลุ่มผู้สูงอายุที่มีภาวการณ์รู้คิดบกพร่องเล็กน้อย (MCI) กลุ่มผู้สูงอายุที่มีภาวะสมองเสื่อมในระยะที่เป็นน้อย (mild Dementia) กลุ่มตัวอย่างเข้ารับการประเมินแบบประเมินภาวะสมองเสื่อมและภาวะการรู้คิดทั่วไป Thai Mental State Examination (TMSE) แบบประเมินภาวการณ์รู้คิด Montreal Cognitive Assessment (MoCA) และ แบบทดสอบทางประสาทจิตวิทยา CERAD battery test และแบบทดสอบทางประสาทจิตวิทยาโดยคอมพิวเตอร์ในศูนย์ผู้ดูแลผู้ป่วยสมองเสื่อม CANTAB ในตัวอย่างทั้ง 3 กลุ่ม นำผลที่ได้มาวิเคราะห์หาค่าสถิติเชิงพรรณนา (Descriptive statistic) เพื่ออธิบายลักษณะทั่วไปของกลุ่มตัวอย่างที่ทำการศึกษา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมาน (correlation statistics) เพื่อดูความสัมพันธ์ของปัจจัยต่าง ๆ ผลการวิจัยพบว่า ในแบบทดสอบทางประสาทจิตวิทยา CERAD battery test กับแบบประเมินความสามารถทางจิตประสาทด้วยคอมพิวเตอร์ (CANTAB) รายข้อ พบว่าค่าสหสัมพันธ์ ในข้อ DMS Percent correct (all delays)’ มีค่าความสัมพันธ์กับกลุ่มตัวอย่างทั้ง 3 กลุ่ม (r=0.13, 0.16, 0.22 ตามลำดับ) สรุปได้ว่า อาจยังมีข้อจำกัดในด้านการเก็บข้อมูลที่ยังมีการประเมินในแต่ละด้านของภาวะรู้คิดไม่มากนัก และควรหาความสัมพันธ์ ในรายละเอียดของแต่ละ cognitive domain ร่วมไปกับการวิเคราะห์ในรายละเอียดคะแนนของ CANTAB จะทำให้สามารถนำค่าคะแนน CANTAB มาใช้ประโยชน์ได้มากขึ้น ในโอกาสต่อไปถ้ามีการศึกษาหาค่า normative ของ CANTAB แต่ละข้อ ในกลุ่มประชากรไทยที่มีการแบ่งตามจำนวนปีการศึกษาและอายุ ในจำนวนประชากรที่มากพอจะช่วยให้มีฐานข้อมูลและนำมาหาความเที่ยงตรงเพื่อนำไปใช้ร่วมกับการวินิจฉัยภาวะการรู้คิดบกพร่องจากโรคต่าง ๆ ได้ อย่างเป็นประโยชน์มากขึ้น
