Abstract Introduction: Femoro-acetabular impingement (FAI) is recognized as one of the major causes of hip pain, labral tears, and early degenerative hip cartilage changes. Little is known about the effectiveness of therapeutic exercises for managing pain, function, and quality of life of patients with FAI. This study examines the effectiveness of a home-based hip exercise program for patients with symptomatic FAI in reducing pain and improving function and quality of life. Method: A single-blind parallel-group randomized controlled trial was conducted. Eligible participants were > 16 years old and diagnosed with symptomatic FAI. A sample size of 84 was needed to detect a clinically important difference. Participants were randomly allocated to 1) an 8-week home-based exercise program focused on posterior pelvic tilt supervised by a physiotherapist every two weeks; and 2) usual care. Outcome measures were function (iHOT-33), quality of life (EQ-5D-5L), and pain (Visual Analog Scale) at baseline, 9 weeks, and 6 months. Independent sample t-tests and chi-squared tests were used to test between-group differences in scores using a level of significance. Results: Ninety-Five participants were recruited (48 intervention, 47 control), 55 were Males (55%) and the mean age was 32 (standard deviation 8.5). Both groups were similar at baseline (age p=0.69, sex p=0.12, function p=0.43, pain p=0.67, quality of life index value p=0.34, quality of life VAS p=0.44). With the number available for the study, there were no statistically significant differences between groups at 9 weeks and 6 months for function (9 weeks p=.53, 6 months p=.78), pain (9 weeks p=.16, 6 months p=.55), and quality of life (index value 9 weeks p=.51, 6 months p=.13; VAS 9 weeks p=.82, 6 months p=.31). Discussion: Results of this study demonstrated no statistical differences in pain, function, and quality of life for patients following an 8-week home-based exercise program focused on posterior pelvic tilt supervised by a physiotherapist compared to usual care. Conclusion: There are no differences in function, quality of life, and pain for patients with FAI completing an 8-week home-based exercise program focused on posterior pelvic tilt supervised by a physiotherapist compared to patients following usual care.
