Abstract Background: Hip dysplasia involves inadequate acetabular coverage, causing excessive rim loading, while increased coverage leads to femoroacetabular impingement (FAI), resulting in labral and cartilage degeneration. However, abnormal acetabular coverage and version are also observed in asymptomatic individuals, making it challenging to distinguish between normal and pathological conditions. The anterior wall index (AWI) and posterior wall index (PWI) can guide diagnosis, but their utilization remains largely unexplored. Questions/purposes: (1) Determine the incidence of each diagnostic category within the population of symptomatic hip pain patients. (2) Assess acetabular parameters between asymptomatic, well-functioning volunteers and patients with symptomatic hip pain. Methods: This is a prospective, single-center study performed at an academic referral center. Patients presenting at the hospital with hip pain without arthrosis (n=288) (mean age: 36 ±9 years, 170 (58%) females) between February 2017 and February 2024 were enrolled. Additionally, one hundred volunteers (students, staff, patients with upper limb injuries) with well-functioning hips (Oxford hip score > 45) (45 (45%) females, mean age 37 ±14 years) were recruited for participation. Supine AP pelvic radiographs were analyzed to determine numerous acetabular parameters including the lateral center-edge angle (LCEA), the acetabular index (AI), the anterior wall index (AWI), the posterior wall index (PWI), the cross over sign (COS), the cross over ratio (COR), the posterior wall sign (PWS), the ischial spine sign (ISS). Differences in acetabular parameters were determined between asymptomatic volunteers and patients with hip pain. Results: In our cohort of hip pain patients without arthrosis, FAI was the most prevalent diagnosis (58%), followed by dysplasia (18%), mixed FAI-dysplasia (9%), labral tear (8%), and acetabular retroversion (7%). Anterior dysplasia presented an AWI of 0.24 ± 0.05 and a PWI of 1.00 ±0.12 with 100% showing an AWI <0.31, while subjects with retroversion exhibited a mean AWI of 0.58 ± 0.10 and a mean PWI of 0.81, with 76% showing an AWI > 0.51 and 48% a PWI <0.81. Asymptomatic subjects had an AWI of 0.47 ±0.13 and a PWI of 0.94 ±0.15. Discussion: The study illuminates the intricate relationship between acetabular morphology and symptomatic hip conditions. Parameters traditionally considered abnormal are frequently observed in asymptomatic individuals.
