Colecistectomía temprana comparada con la tardía en pacientes con pancreatitis aguda biliar moderada a severa: Revisión sistemática

Abstract
Introducción: La pancreatitis aguda biliar (PAB) es una de las principales causas de inflamación pancreática, asociada principalmente a cálculos biliares. Su incidencia global ha aumentado en las últimas décadas. Según los criterios de Atlanta, la PAB se clasifica en leve, moderada y severa, siendo la forma severa la más asociada a altas tasas de mortalidad y complicaciones. La colecistectomía laparoscópica es un pilar en el manejo de la PAB para prevenir recurrencias; sin embargo, el momento óptimo para realizarla sigue siendo controversial, especialmente en casos moderados y severos (PABMS). Metodología: Se realizó una revisión sistemática siguiendo los lineamientos PRISMA y el Manual Cochrane, con registro en PROSPERO (CRD42024600492). Se incluyeron ensayos clínicos aleatorizados (ECA) y estudios observacionales con grupo comparador que evaluaran la eficacia y seguridad de la colecistectomía temprana (dentro de las tres semanas) frente a la tardía en pacientes con PABMS. La búsqueda bibliográfica se realizó en MEDLINE, EMBASE, SCOPUS, Web of Science y Cochrane CENTRAL, sin restricciones de idioma ni fecha de publicación. Dos revisores seleccionaron y extrajeron los datos de forma independiente, y se evaluó el riesgo de sesgo con herramientas específicas para cada diseño (RoB 1 y Newcastle-Ottawa). Los resultados se sintetizaron cualitativamente debido a la heterogeneidad metodológica y las diferencias en las definiciones de intervención temprana y tardía. Resultados: Se incluyeron 2 estudios (1 ECA y 1 cohorte retrospectiva) con 3904 pacientes. En PAB moderada, no hubo diferencias significativas en la recurrencia de eventos biliares entre CTe y CTa (0% vs. 4%, p = 0.121), aunque se observó una tendencia favorable hacia la CTe.
