A suplementação de vitamina D tem sido amplamente investigada por seu impacto na saúde óssea, mas estudos recentes sugerem que ela pode desempenhar um papel importante na performance física e recuperação de atletas de resistência, incluindo ultramaratonistas. Este estudo teve como objetivo analisar a influência dos níveis de vitamina D na performance e recuperação de ultramaratonistas por meio de uma revisão sistemática com meta-análise. Foram selecionados ensaios clínicos randomizados (ECRs) e estudos observacionais que investigaram os efeitos da suplementação de vitamina D em atletas de ultramaratona, com ênfase em parâmetros como resistência, recuperação muscular e prevenção de lesões. A qualidade dos estudos foi avaliada utilizando ferramentas padronizadas como a Risk of Bias Tool da Cochrane e a Newcastle-Ottawa Scale (NOS), com a maioria dos estudos apresentando qualidade moderada a alta. A meta-análise indicou que a suplementação de vitamina D (1000 a 4000 UI/dia) teve efeitos positivos significativos na melhoria da resistência física (10,2% de aumento médio) e na aceleração da recuperação muscular, com uma redução de 12,3% no risco de lesões musculares. Além disso, doses mais altas e intervenções mais longas (acima de 8 semanas) demonstraram melhores resultados. A qualidade da evidência foi classificada como moderada para os efeitos sobre resistência e recuperação, e baixa para a prevenção de lesões.
