Abstract
A 10-year-old boy was referred to a pediatrician for a 2-month history of episodic abdominal pain, nausea and vomiting, and intermittent hematochezia and black stools, interspersed with normal bowel movements. He had no diarrhoea, fevers, rashes or travel history, and review of systems was otherwise negative. His past medical history was unremarkable. Family history identified thalassemia trait in the boy’s father but no GI or autoimmune illnesses. The boy’s initial haemoglobin was 102 g/L and he was started on oral iron supplementation. Other investigations included TTG (<0.5 U/mL), faecal calprotectin (<19 ug/g), CRP (<0.6 mg/L) and albumin (45 g/L). FOBT was negative. He was diagnosed with constipation shortly after his initial paediatrics visit. A 2-week course of stool softener led to worsening of his abdominal pain and nausea so this was discontinued. He was referred for consultation with paediatric gastroenterology (GI). At the time of GI consultation 2 months later, his haemoglobin was 56 g/L with ferritin 14 ug/L. During the time leading up to GI consultation, his haemoglobin ranged between 56 and 80 g/L despite oral iron. He continued to have abdominal pain, and intermittent hematochezia and black stools. The black stools were difficult to interpret since he was taking oral iron. He was fatigued but had no new symptoms. Shortly after seeing GI, he received an iron infusion and subsequently endocolonoscopy with normal biopsies. Capsule endoscopy was then pursued but did not identify any bleeding source.
