Preeclampsia como factor de riesgo para hemorragia postparto: Una revisión sistemática con Metaanálisis

Abstract

Objetivos. El sangrado postparto se considera como una de las principales desencadenantes de la mortalidad materna. La preeclampsia podría aumentar el riesgo de hemorragia postparto debido a la disfunción endotelial y alteración vascular. Aunque varios estudios sugieren esta asociación, aún no está claramente establecida, por ello, realizamos una revisión sistemática con metaanálisis basados en estudios observacionales para evaluar el impacto de la preeclampsia sobre la hemorragia postparto. Métodos. Esta revisión sistemática siguió las pautas PRISMA. El resultado primario fue la hemorragia postparto. Se incluyeron estudios observacionales (casos y controles y estudio de cohortes). Se planteo análisis de subgrupos por vía de parto y procedencia por continente. La evaluación de riesgo de sesgo se realizó utilizando la escala de Newcastle – Ottawa. (número de registro de la base de datos PROSPERO: CRD42023441579). Resultados: Se incluyeron 12 estudios observacionales (7 casos y controles, 5 estudios de cohortes retrospectivas) con un total de 2,102,987 gestantes, de las cuales 61139 presentaron preeclampsia. La preeclampsia se relacionó con un incremento del riesgo de hemorragia postparto (OR 1.95, IC 95% 1.70-2.23). El análisis por subgrupos por continente, mostro un OR de 2.18 para los estudios realizados en Europa, un OR de 1.95 para los estudios realizados en América y un OR de 1.67 para los estudios realizados en Asia. La heterogeneidad estadística fue alta (I²=67.5%), la certeza de la evidencia fue moderada según la valoración GRADE. Se identificaron sesgos en algunos estudios debido a la falta de ajuste por variables confusoras Conclusiones. Los resultados sugieren que la preeclampsia se relaciona con un incremento del riesgo de hemorragia postparto con una certeza moderada de la evidencia y bajo riesgo de sesgo.
