Abstract This chapter reviews the epidemiology, diagnosis, physiopathology, and treatment of shift-work disorder irregular and non-24-hour sleep–wake rhythm disorders. Shift-work disorder is caused by self-selected behavioral changes due to an atypical work schedule, whereas the other disorders are presumably caused by an intrinsic malfunction of the endogenous circadian timing system. The clinical management of these disorders requires long periods of investigation and observation. Various countermeasures such as improved light/dark exposure, sleep management, and careful use of hypnotics and stimulants when needed can be considered in shift workers with severe sleep–wake disturbances. For the other disorders, therapeutic decisions should seek to consolidate sleep at night; and a multimodal approach is suggested, although no clear guidelines are available. Exogenous melatonin or melatonin agonists are recommended for free-running patients with blindness. In all cases, a careful review of sleep hygiene is important as maladaptive behaviors are frequent.
