Introduction: Severe stages of Obesity increased cardiovascular and mortality risk, especially in people with Diabetes Mellitus. Last choice is a bariatric surgery which is permits to decrease the risk and prolong life longevity and quality. We interested how body composition will changes in patients after bariatric surgery. Material and methods. Anthropometric data, blood fasting and after meal glycemia, HbA1c, blood AlT, AsT, total cholesterol level was observed before and after bariatric Y and Rouge surgery in 42 patients with type 2 DM. Body compositions were assessed by bioimpedance method and calculated as a percentage to body mass. Results. Body mass indexes were 42 m 2 /kg and decreased by 34% (p<0,05) after bariatric surgery. Blood fasting (67%) and 2h after (59%) meal glycemia level, as well as HbA1c (75%), total blood cholesterol level (73%) were significantly decreased, whereas AlT, AsT were in normal range and remain unchanged. Interesting that 54% of patients blood glucose control were normal and they stop sugar lowering medications, others continue previous hypoglycemic therapy, but dosages were decreased more than 2 times. Body composition before surgery were increased by fat masses, 66% of body fat in average and were decreased until 42% (p<0,05) of body mass after 3 months of bariatric surgery. Lean masses were decreased and composed16% in averages before surgery, which gained until 21% after surgery. Bone mass were decreased before surgery and significantly increased after surgery and accompanied by increasing of blood platelets numbers and fib4 score. That finding indicate that bone metabolism is affected probably through FGF23 and klotho protein pathway. Whereas body water mass was significantly decreased after surgery, which is need to further investigation regarding water replacement, especially in patients with DM2. Conclusion. Bariatric surgery in severe obese type 2 DM patients significantly improved body fat and lean masses, blood glycemia, HbA1c and total cholesterol level, whereas body fluids and bone metabolism require more deeper investigations.
