Abstract
Résumé Les personnes présentant une kinésiophobie élevée risquent plus de développer douleurs chroniques et handicaps persistants, mais leur impact sur le système moteur reste peu clair. Nous avons étudié si la kinésiophobie module les effets de la douleur à l’épaule sur (i) la cinématique et l’activation musculaire durant un mouvement fonctionnel, et (ii) l’excitabilité corticospinale. Trente sujets sains ont réalisé une tâche de pointage avant et pendant une douleur induite par capsaïcine à l’épaule. Cinématique de l’épaule, coude, doigts, et l’électromyographie du trapèze supérieur et deltoïde antérieur ont été enregistrés. L’excitabilité corticale a été mesurée via la pente des courbes entrée-sortie en utilisant la stimulation magnétique transcrânienne. La douleur a réduit l’activité électromyographique de l’épaule et a varié la cinématique digitale, avec une réduction plus marquée chez les sujets kinésiophobes. Le score de kinésiophobie corrélait aux modifications de l’excitabilité corticospinale, suggérant une influence précoce sur l’activité motrice. Ces résultats indiquent une interaction entre douleur, kinésiophobie et contrôle moteur.
