Einfluss präoperativer mentaler Gesundheit auf das postoperative Ergebnis und die Lebensqualität nach anatomischer Lungenresektion

Abstract
Hintergrund Neben funktionellen und operativ-technischen Aspekten ist die mentale Gesundheit entscheidend für die Erholung und Lebensqualität nach operativen Maßnahmen. Systematische Untersuchungen zum Einfluss von Faktoren wie Angst, Stress und Depressionen auf postoperative Ergebnisse bei thoraxonkologischen Eingriffen sind bisher nicht ausreichend erfolgt. Material und Methode Es handelt sich um eine prospektive Kohortenstudie. Eingeschlossen wurden Patienten, bei denen zwischen April 2023 und Juni 2024 eine anatomische Lungenresektion ohne komplizierende Faktoren gemäß der Ein- und Ausschlusskriterien durchgeführt wurde. Angst, Depression, Stress und Lebensqualität wurden präoperativ, bei Entlassung aus der stationären Behandlung sowie 3, 6 und 12 Monaten postoperativ mithilfe von Fragebögen (PSS-10, HADS und EORTC QLQ-C30 und LC13) erhoben. Ergebnis Es wurden insgesamt 98 Patienten in die Studie eingeschlossen. In der vorläufigen Analyse der ersten 53 Patienten zeigten sich präoperativ erhöhte Stress- (PSS-10 >23) und Angst-Scores (HADS >7) in 28 (53%) bzw. 19 Patienten (36%), erhöhte Depressionswerte (HADS >7) wurde in 8 Patienten (15%) beobachtet. Präoperativ erhöhte Angst-Scores korrelierten mit erhöhten Depressions- (p=0,004) und Stress-Scores (p<0,0001). Zudem konnten wir eine quantitative Assoziation zwischen erhöhten präoperativen Angstscores und postoperativen Komplikationen beobachten (Ottawa Classification ≥IIIa; p=0,3) Präoperativer Stress korrelierte nicht mit postoperativen Komplikationen (p=0,5). Der Global Health Score nahm in der Gesamtkohorte postoperativ zunächst ab, erholte sich aber nach 6 Monaten auf sein Ausgangsniveau. Patienten mit präoperativ erhöhtem Depressionsscore zeigten direkt postoperativ einen signifikant niedrigeren Global Health Score als Patienten mit unauffälligen Depressionsscores (p=0,03) mit weiterhin erniedrigten Global Health Scores während der gesamten Nachverfolgungszeit. Präoperativ erhöhte Angst- und Stress-Level scheinen keinen Einfluss auf die langfristige Lebensqualität zu haben. Schlussfolgerung Ein relevanter Anteil an Patienten, welche sich einer anatomischen Lungenresektion unterziehen, weist präoperativ Aspekte einer beeinträchtigten mentalen Gesundheit auf, welche sich negativ auf das postoperative Ergebnis und die Lebensqualität auswirken. Diese Ergebnisse unterstreichen die Bedeutung frühzeitiger psychoonkologischer Interventionen, beispielsweise im Rahmen von Prähabilitationsmaßnahmen.
