Rol de los linfocitos T en la asociación entre la diabetes y la periodontitis: una revisión sistemática

Objetivo: Determinar el rol de los linfocitos T en la asociación entre la diabetes y la periodontitis, tanto en humanos como en animales, mediante una revisión sistemática. Métodos: La búsqueda electrónica se realizó en cuatro bases de datos: Medline, Scopus, Embase y Web of Science hasta el 30 de enero de 2025, con una búsqueda manual complementaria. El análisis de riesgo de sesgo se realizó con las herramientas de Newcastle-Ottawa para estudios en humanos y SYRCLE para estudios en animales. Resultados: Un total de 14 de los 675 estudios identificados en las bases de datos fueron incluidos. Cuatro estudios en animales asociaron la presencia y el número de linfocitos T a una mayor pérdida ósea en modelos de periodontitis y diabetes. Once estudios en humanos analizaron la función de los linfocitos T en sangre y fluido crevicular, reportando una asociación entre el desarrollo de la periodontitis en relación con la diabetes mellitus tipo 1 o 2, influenciado por la actividad entre linfocitos Th17, T reguladores, y sus citoquinas, determinando en específico la resorción ósea durante la periodontitis. Una cantidad de 8 estudios en humanos presentaron bajo riesgo y 3 alto riesgo de sesgo, mientras que 3 estudios en animales presentaron alto riesgo y solo 1 estudio mostró bajo riesgo de sesgo. Conclusiones: Los cambios en la cantidad y función de los linfocitos T pueden agravar la periodontitis en pacientes diabéticos.
