63285 Bimekizumab efficacy by patient subgroups in moderate to severe hidradenitis suppurativa: 2-year phase 3 results from BE HEARD EXT

Abstract
Introduction Response to hidradenitis suppurativa (HS) treatment can vary depending on patient characteristics, necessitating new treatments with consistent efficacy.1 Bimekizumab is a humanized IgG1 monoclonal antibody that inhibits interleukin (IL)-17F in addition to IL-17A registered for the treatment of HS.2 Here, we report bimekizumab efficacy outcomes across different subgroups in patients with moderate to severe HS over 3 years. We aim to assess the maintenance of clinical response over 3 years with bimekizumab by age, sex, disease duration, Hurley Stage, and body mass index (BMI). Procedure/Study Data were pooled from the BE HEARD I&II (BHI&II) studies and their open-label extension, BE HEARD EXT (BHEXT).3,4 Proportions of patients achieving ≥50%/75% HiSCR (HiSCR50/75) in subgroups (age [<35years, ≥35years], sex, disease duration at baseline [<5.02years, ≥5.02years (median)], Hurley Stage [II, III], BMI [<30kg/m2, 30–<35kg/m2, ≥35kg/m2]) are reported at Year1 (Week48) and Year3 (Week148). Data reported for patients randomized to bimekizumab from baseline (Bimekizumab Total) in BHI&II who entered BHEXT (observed case). Results 657 BHI&II completers entered BHEXT; 556 received bimekizumab from baseline. Overall, a high proportion of patients reached HiSCR50/75 at Year1 (n=444/556 [79.9%]/n=356/556[64.0%]) and Year3 (n=331/367[90.2%]/n=298/367[81.2%]). High proportions of patients reached HiSCR50/75 across subgroups at Year1: <35years, 80.5%/65.2%, ≥35years, 79.2%/63.0%; male, 80.5%/63.4%, female, 79.3%/64.5%; median disease duration <5.02years, 83.2%/66.3%, ≥5.02years, 76.4%/61.6%; Hurley Stage II, 83.5%/69.6%, Hurley Stage III, 75.5%/57.3%; BMI <30kg/m2, 81.2%/67.9%, BMI 30–<35kg/m2, 81.3%/63.4%, BMI ≥35kg/m2, 77.1%/59.6%. At Year3, proportions reaching HiSCR50/75 were further increased or maintained across subgroups: <35years, 91.7%/83.4%, ≥35years, 88.9%/79.3%; male, 89.3%/78.5%, female, 91.1%/83.7%; median disease duration <5.02years, 89.9%/80.9%, ≥5.02years, 90.5%/81.5%; Hurley Stage II, 87.5%/81.7%, Hurley Stage III, 88.5%/80.6%, BMI <30kg/m2, 92.0%/81.5%, BMI 30–<35kg/m2, 87.2%/80.2%, BMI ≥35kg/m2, 89.9%/81.5%. Conclusion Patients treated with bimekizumab demonstrated efficacy in all subgroups with improvements in clinical response maintained over 3 years.
