Measurement of Age-Dependent Z-Scores for Cerebellar Tonsil Position in Pediatric Controls Compared to Children with Chiari Type 1 Malformation

Chiari Malformation Type 1 (CM1) is a structural neurological condition characterized by the downward displacement of the cerebellar tonsils through the foramen magnum. The current diagnostic threshold (a tonsillar descent ≥ 5 mm) does not account for age-related anatomical variation, which can be particularly problematic in pediatric populations. This study aimed to develop age-adjusted Z-scores for cerebellar tonsil position and evaluate their diagnostic utility in distinguishing CM1 from normal anatomical variations. A retrospective analysis of MRI scans from 151 pediatric controls and 105 CM1 patients aged 2 to 18 was conducted. Cerebellar tonsillar position was measured using standardized sagittal-plane techniques and normative data from controls was used to calculate age-dependent Z-scores. Z-score distributions were compared between groups, and subgroup analyses were performed based on surgical intervention and the presence of syringomyelia. Tonsillar descent increased with age in the control group, while CM1 patients exhibited persistent descent across ages. Z-scores in controls followed a normal distribution centered around 0, while CM1 patient scores were significantly right-shifted. A cutoff of Z ≥ 2 yielded a 96.1% sensitivity and a 97.4% specificity, with surgical patients showing significantly higher Z-scores than non-surgical patients, while no significant difference in Z-scores were observed between CM1 patients with and without syringomyelia. Overall, age-adjusted Z-scores offer a more accurate and individualized approach to assessing cerebellar tonsillar position in pediatric patients. This method improves diagnostic precision over fixed thresholds and may guide clinical decision-making in CM1 diagnosis and treatment.
