Kann “Food Addiction“ den Erfolg gewichtsreduzierender Behandlungen bei Übergewicht und Adipositas vorhersagen? Eine systematische Übersichtsarbeit und Metaanalyse mit Fokus auf Aspekten der Diversität

Abstract
Einleitung : Suchtartiges Essverhalten (Food Addiction, FA) gilt als möglicher prognostischer Faktor für den Erfolg gewichtsreduzierender Behandlungen bei Übergewicht/Adipositas. Ziel dieser systematischen Übersichtsarbeit mit Metaanalyse war es, (1) die prognostische Wirkung von FA-Status und -Schweregrad bei Personen mit Übergewicht oder Adipositas auf die Ergebnisse einer Gewichtsreduktionsbehandlung zu bewerten und (2) potenzielle Moderatoren des Zusammenhangs auf der Grundlage von Interventions-, Studien- und Stichprobenmerkmalen zu untersuchen. Methoden : Systematische Suche in PubMed, PsycINFO, Web of Science nach Studien, die Zusammenhänge zwischen FA und Ergebnissen gewichtsreduzierender Behandlungen (z.B. bariatrische Operation) bei Personen mit Übergewicht oder Adipositas ohne Essstörungen berichteten. Korrelationen zwischen FA-Schweregrad und Gewichtsreduktion bzw. Effektstärken der Gruppenvergleiche (FA positiv vs. negativ) wurden extrahiert und mittels „Random-Effects“ Modellen ausgewertet. Ergebnisse : Fünfundzwanzig Studien (N=4904 Personen) erfüllten die Einschlusskriterien. Hieraus berechneten wir k=18 Korrelationen zwischen Gewichtsverlust und FA-Schweregrad und k=21 Effektgrößen für den Vergleich der Gewichtsabnahme zwischen den FA-Gruppen. Die Metaanalysen zeigten negative, aber nicht signifikante Zusammenhänge zwischen dem Schweregrad oder der Diagnose von FA und dem Gewichtsverlust. Die Zusammenhänge fielen stärker aus für verhaltenstherapeutische Maßnahmen zur Gewichtsabnahme und in ethnisch diversen Stichproben. Schlussfolgerung : Weitere Untersuchungen über die Wechselwirkung von FA mit bestehenden psychischen und sozialen Vulnerabilitäten sind geboten.
