O câncer, caracterizado pela proliferação desordenada de células, pode gerar impactos significativos na saúde física, mental e cognitiva dos indivíduos. Dentre os efeitos adversos do tratamento oncológico, destacam-se alterações cognitivas, como prejuízos na fluência verbal, que afetam funções executivas, memória e atenção. Essas alterações, denominadas "chemobrain", impactam a qualidade de vida dos pacientes oncológicos. Diante disso, este estudo teve como objetivo analisar o desempenho de fluência verbal em adultos e idosos com câncer após tratamento quimioterápico. Para tal, foi realizado um estudo de caso-controle, quantitativo, descritivo e transversal, realizado em um hospital filantrópico. A amostra foi composta por 60 indivíduos, divididos em dois grupos pareados por idade, sexo e escolaridade: 30 participantes com diagnóstico de câncer em tratamento exclusivo com quimioterapia e 30 participantes sem histórico da doença. Os instrumentos aplicados foram um questionário sociodemográfico e clínico, o Montreal Cognitive Assessment (MoCA), e os testes de fluência verbal semântica (categoria “animais”) e fonológica (letra “A”), ambos com tempo de 60 segundos. Os dados foram analisados por meio de estatística descritiva e inferencial. Os tipos de câncer mais prevalentes foram estômago (20%), mama (13,3%) e linfonodo (13,3%). Os quimioterápicos mais utilizados foram fluorouracil (20%) e cisplatina (14%). Os participantes sem câncer apresentaram escores superiores na fluência verbal semântica e fonológica (p<0,001) em comparação aos pacientes em tratamento. Não houve diferença estatística nos escores do MoCA entre os grupos. No grupo com câncer, houve correlação fraca entre fluência fonológica e escore MoCA (p=0,04). Já no grupo controle, observaram-se correlações moderadas entre fluência verbal semântica (p=0,007) e fonológica (p=0,016) e MoCA; e correlação negativa moderada entre fluência fonológica e idade (p=0,021). Conclui-se que os indivíduos com câncer em tratamento quimioterápico apresentam desempenho inferior nas tarefas de fluência verbal semântica e fonológica em comparação com pessoas sem histórico da doença.
