Primärer Wundverschluss vs. sekundäre Wundheilung von Pin-Stellen nach temporärer Fixateurentfernung: Ergebnisse einer randomisierten Studie

All articles of this category Die prospektive, randomisierte und verblindete Monozentrumsstudie aus Luzern untersucht, ob der primäre Wundverschluss von Pin-Stellen nach Entfernung eines temporären Fixateurs im Vergleich zur offenen sekundären Wundheilung zu einer höheren Infektionsrate führt. Der Fokus liegt auf Patienten, die an den oberen oder unteren Extremitäten mit temporären Fixateuren behandelt wurden. Insgesamt wurden 70 Patienten mit 241 Pin-Stellen in die Studie eingeschlossen. Die randomisierte Zuteilung erfolgte auf Wundebene: Jeweils die proximalste Pin-Stelle wurde im Verhältnis 1:1 entweder primär verschlossen oder offen belassen. Die restlichen Pin-Stellen wurden alternierend behandelt. Der primäre Endpunkt war die Rate an Pin-Stellen-Infektionen. Sekundäre Endpunkte umfassten Komplikationen, Zeit bis zur Wundheilung, Patientenzufriedenheit, Schmerzscore (VAS) sowie die Vancouver Scar Scale (VSS). Ergebnisse zeigten keine Pin-Stellen-Infektionen in beiden Gruppen. Die Wundheilung verlief signifikant schneller nach primärem Verschluss (Median: 2 Wochen vs. 6 Wochen, p = 0,013). Unterschiede in Narbenqualität, Schmerzempfinden oder Zufriedenheit waren nicht signifikant. Subgruppenanalysen ergaben einen signifikanten Vorteil des primären Verschlusses nur an den unteren Extremitäten. Limitationen der Studie sind das Monozentrumsdesign, eine niedrige Komplikationsrate sowie eine mögliche Übertragungsproblematik auf andere Patientengruppen (z. B. immunsupprimierte Patienten oder Pin-Stellen an der Ferse, die nicht eingeschlossen wurden). Dennoch liefert die Studie erstmalig evidenzbasierte Daten zu einem bislang kaum untersuchten Thema. Die Autoren schlussfolgern, dass der primäre Verschluss nach sorgfältigem Débridement und Spülung eine sichere Option darstellt, die zu schnellerer Heilung führt, ohne die Infektionsrate zu erhöhen. Die Studienergebnisse sprechen dafür, den primären Wundverschluss bei nicht immunsupprimierten Patienten nach temporärer Fixateuranlage zumindest in Erwägung zu ziehen.
