Examining conceptions of hope to inform development of strength-based activities to prevent type 2 diabetes among Indigenous youth

Abstract
BACKGROUND: In Canada, Indigenous peoples are disproportionately affected by type 2 diabetes (T2D) and related complications due to the lasting impacts of colonization, cultural dispossession, and social inequities. Early onset of T2D is associated with various long-term co-morbidities, emphasizing the importance of efforts to support the health and well-being of Indigenous youth with the aim to prevent T2D. To address disproportionate rates of T2D among Indigenous youth, Indigenous community partners and our research team are co-designing culturally-informed health resources and activities. To guide development, we conducted qualitative research in a mid-western Canadian province to explore Indigenous Elders, Knowledge Keepers, and adults' conceptions of hope relative to wellness and T2D prevention. METHODS: The authors adopted Ermine's 'ethical space' orientation and a community-engaged approach whereby a purposive sample of Indigenous adults (n = 29), Elders, and Knowledge Keepers (n = 7) participated in qualitative focus groups (n = 6) and interviews (n = 7). The resultant data were analyzed thematically. RESULTS: Three themes and associated sub-themes were identified: Hope for Enhanced Education (What is Diabetes?, Visions for Diabetes Education), Structural Supports in Communities (Enhancing the Food Environment, Creating Opportunities for Physical Activity), and Traditional Ways (Practicing Traditional Ways to "Keep You in Balance", Intergenerational Teachings: Showing the Way). CONCLUSIONS: Participants' hopes highlight tangible actions for enhanced diabetes education, community supports, and engagement in traditional ways as means for promoting health and preventing T2D among Indigenous youth from the communities. These findings will inform a strength-based approach within health promotion and T2D prevention resources and activities that complement existing initiatives.
