Abstract
Mục tiêu: Khảo sát đặc điểm lâm sàng, cận lâm sàng và tỷ lệ tổn thương hậu môn ở bệnh nhân Crohn. Đối tượng và phương pháp nghiên cứu: nghiên cứu mô tả cắt ngang trên 140 bệnh nhân điều trị tại bệnh viện Bạch Mai và Đại học Y Hà Nội từ tháng 8/2024 đến tháng 5/2025. Kết quả: Tuổi trung bình của nghiên cứu là 31,66, nam giới chiếm 51,4%. Triệu chứng lâm sàng chủ yếu của bệnh Crohn là đau bụng, rối loạn phân và đi ngoài phân máu. Theo phân loại Montreal, Crohn thường gặp ở hồi đại tràng và đại tràng, ít gặp Crohn đường tiêu hoá trên. Tỷ lệ tổn thương hậu môn là 42,9%. Rò quanh hậu môn (78,3%) và áp xe hậu môn (50%) là tổn thương hậu môn phổ biến nhất. Giới tính nam, tuổi < 40 có mối liên quan đến tỷ lệ cao mắc tổn thương hậu môn, trong khi đó tổn thương ở hồi tràng, tổn thương hẹp trên nội soi liên quan đến tỷ lệ thấp mắc tổn thương hậu môn. Kết luận: Bệnh Crohn có biểu hiện lâm sàng đa dạng, trong đó tổn thương hậu môn là một trong các biểu hiện của bệnh. 50% bệnh nhân có tổn thương hậu môn đã từng phải trải qua phẫu thuật vì biến chứng này. Tuổi trẻ, giới tính nam, vị trí và mức độ tổn thương có mối liên quan có ý nghĩa thống kê với tỷ lệ mắc tổn thương quanh hậu môn.
