Abstract
Введение. Изучение психологических характеристик пациентов с хроническим болевым синдромом является важным этапом совершенствования оказания медицинской помощи, учитывая их значимое влияние на восприятие и выраженность боли. Цель. Изучить психологические характеристики пациентов с хроническим болевым синдромом в сравнении с группой пациентов без хронического болевого синдрома. Материалы и методы. Анкетирование 123 пациентов с хроническим болевым синдромом различной этиологии и 33 добровольцев без признаков хронической боли. Для оценки боли заполнялся опросник PainDetect. Психологические характеристики изучались с помощью шкалы оценки депрессии A.T. Beck 1996 г., теста оценки тревожности Спилбергера, Торонтской шкалы алекситимии (TAS-20R), Шкалы катастрофизации боли (Pain Catastrophizing Scale – PCS). Результаты. По данным опросника PainDetect, для пациентов основной группы характерно наличие болевого синдрома выраженной степени и высокой вероятности невропатического компонента боли. В группе контроля выявлена боль легкой или умеренной степени (U=140,5; p<0,0001) и отсутствуют признаки невропатической боли (U=92,0; p<0,0001). В основной группе выявлены значимое преобладание алекситимических черт (U=1042,5; p=0,02), выраженность депрессии (U=706,5; p=0,0003), склонность к катастрофизации (U=192,0; p<0,0001). Обе группы показали средний уровень тревожности, однако выраженность ситуационной тревоги достоверно отличалась в основной группе (U=776,5; p=0,0013). Заключение. Для пациентов с хроническими болевыми синдромами характерны повышенная алекситимичность и склонность к катастрофизации. Клинически значимые признаки депрессии выявлены у 44% пациентов. 97,2% пациентов основной группы имеют средний или высокий уровень как ситуационной, так и личностной тревоги. Изучение психологических особенностей пациентов с хронической болью является важной составляющей для формирования индивидуального плана терапии. Materials and methods. Questioning of 123 patients with chronic pain syndrome of various etiologies and 33 volunteers without signs of chronic pain was performed. Pain was assessed using the PainDetect questionnaire. Psychological characteristics were studied using the A.T. Beck Depression Rating Scale of 1996, the Spielberger Anxiety Rating Test, the Toronto Alexithymia Scale (TAS-20R), and the Pain Catastrophizing Scale (PCS). Results. According to the PainDetect questionnaire, patients in the main group were characterized by severe pain syndrome and a high probability of a neuropathic component of pain. In the control group, mild or moderate pain was detected (U=140,5; p<0,0001) with no signs of neuropathic pain (U=92,0; p<0,0001). In the main group, compared with the control group, a significant predominance of alexithymic traits (U=1042,5; p=0,0287), severity of depression (U=706,5; p=0,0003), and a tendency to catastrophize (U=192,0; p<0,0001) were revealed. Both groups showed an average level of anxiety, but the severity of situational anxiety was significantly different in the main group (U=776,5; p=0,0013). Conclusion. Patients with chronic pain syndromes are characterized by increased alexithymia and a tendency to catastrophize. Clinically significant signs of depression were detected in 44% of patients. 97,2% of patients in the main group presented a moderate or high level of both situational and personal anxiety. Studying psychological characteristics of patients with chronic pain is an important component for forming an individualized therapy plan.
