Abstract FR578: Pediatric Hypertension Guidelines and Early Cardiovascular Risk: A Systematic Review of Evidence and Implications

Introduction: Pediatricians are realizing that hypertension in children is more common, recent guidelines now advise lower blood pressure (BP) thresholds since high BP in youth often leads to early heart problems and may continue into adulthood. We conducted a review to gather information linking children's BP to other cardiovascular risk factors and how these influences everyday medical care. Hypothesis: High BP in children and adolescents brings about a dose-dependent progression of hidden changes in the heart that set specific paths toward cardiovascular disease in adulthood. However, early treatment can reverse these heart changes and impact future cardiovascular risk. Methods: We followed PRISMA that report results related to children's and adolescents' cardiovascular health due to high blood pressure. Three randomized controlled trials and ten observational studies were included out of all the papers found. They studied left ventricular hypertrophy, arterial stiffening or thickening, progression to hypertension in adulthood, and results of treatment or lifestyle changes in these youth. Cochrane Risk of Bias and Newcastle-Ottawa Scale were used to judge the quality of the studies. Results: Researchers observed that children with high blood pressure were more likely to develop early signs of damage to important organs. Those with hypertension during youth showed more left ventricular mass, thicker carotid artery walls, and poorer vasodilation in their arteries compared to youth without hypertension. Besides, continuous monitoring showed that higher blood pressure among children, including the high-normal kind, raised their risk of hypertension in adulthood and the metabolic syndrome. Only a few RCTs have shown that interventions like weight loss, better diet, or hypertension drugs in young people can lower their blood pressure and sometimes reverse signs of left ventricular hypertrophy or improve blood vessels. Altogether, the evidence suggested a significant link between blood pressure in children and a danger of heart problems in the future. Conclusions: The systematic review finds that children with high blood pressure are more likely to develop early indications of heart problems and may be at greater future risk. These findings suggest doctors should be encouraged to find and treat hypertension early. The review points out that watching blood pressure and acting early can help prevent hypertension in children from becoming heart problems in adults.
