SAT-123 Dietary Intake in Women with and without Polycystic Ovary Syndrome Participating in a Lifestyle Intervention Study

Polycystic ovary syndrome (PCOS) affects 6-12% of women of reproductive age and is the leading cause of female infertility. Obesity, present in more than 30% of affected women, exacerbates metabolic complications, making lifestyle modifications the first line of treatment. Objective: 1) To compare the dietary intake and baseline characteristics of women with polycystic ovary syndrome (PCOS) to those without PCOS, participating in a multicenter randomized controlled lifestyle trial. 2) To examine the associations between dietary intake and characteristics and moderating effects of PCOS. Methods: Dietary variables (web-based food frequency questionnaire) and basic characteristics (sociodemographic questionnaire and anthropometric measurements) were compared between groups using t-tests and chi-square tests. Correlations were performed to explore the associations between dietary variables and baseline characteristics: Spearman’s for levels of education (4 ordinal categories analyzed as a continuous variable) and Pearson’s for the others. Interactions with PCOS status were tested using multiple linear regressions. Results: As of November 2024, 128 patients were participating in the study and had complete data (PCOS: 74; Non-PCOS: 54). The mean age was 30.3 years in the PCOS group vs. 32.2 years in the non-PCOS group (p = 0.02). The majority of women had a college-level education (33.8% PCOS vs 42.6% Non-PCOS, p = 0.31), and a body mass index (BMI) > 35 kg/m² (66.2% vs. 77.8%, p = 0.15). Energy intake, macronutrients, micronutrients, and daily portions from various food groups were similar between groups. Significant negative associations were observed between education level and energy intake (Rho = -0.23), protein intake (Rho = -0.24), carbohydrate intake (Rho = -0.24), calcium intake (Rho = -2.91), and dairy product intake (Rho = -0.32). Education level and age were significantly and positively associated with plant protein intake (Rho = 0.24 and r = 0.19, respectively). BMI was significantly associated with the proportion of energy from protein (r = 0.28). In addition, the associations of education with energy (-), carbohydrates (-), fruit and vegetable servings (+), and fruit servings alone (+) were significantly stronger in women with PCOS compared with those without PCOS (significant interactions with PCOS status). Conclusion: No significant differences were found in anthropometric characteristics, sociodemographic factors, and dietary intake between women with and without PCOS. However, notable associations between education level, age, BMI, and some dietary variables were observed, and the associations with education level were generally stronger in women with PCOS compared to non PCOS. This suggests that a higher level of education, and perhaps more awareness, may have a greater impact on lifestyle for women with PCOS. Presentation: Saturday, July 12, 2025
