A insônia, sono em quantidade insuficiente ou de baixa qualidade, tornou-se um problema relevante na sociedade moderna. Diversos medicamentos estão disponíveis para o seu tratamento, com destaque para o Zolpidem. Todavia, efeitos colaterais adversos de seu uso têm sido relatados, entre eles o suicídio. Assim, o objetivo da presente pesquisa foi compreender a relação entre o uso de Zolpidem e o suicídio. Para tanto, desenvolveu-se uma revisão sistemática da literatura com base nas diretrizes PRISMA. Foram encontrados 124 estudos nas bases de dados Pubmed e Cochrane. Descartando-se aqueles que não se enquadraram nos critérios de seleção, sete estudos foram incluídos neste trabalho. Na análise da qualidade dos estudos caso-controle e coorte foi utilizada a escala Newcastle-Ottawa (NOS). As evidências apontam para uma associação significativa entre o uso de Zolpidem e o suicídio. Alguns trabalhos não levaram em consideração fatores importantes, como dosagem do medicamento, duração do tratamento, existência de doenças subjacentes e histórico e severidade de distúrbios psiquiátricos, além de outras comorbidades. Quanto à conduta clínica, ressaltou-se a necessidade do uso cuidadoso do Zolpidem, isto é, em quantidades adequadas, dentro dos limites das dosagens recomendadas para cada grupo e por curto período, não sendo recomendado uso contínuo. Não obstante, as pesquisas selecionadas não conseguiram confirmar a causalidade direta entre Zolpidem e o suicídio e não puderam identificar o mecanismo exato do fenômeno, tópicos que merecem ser abordados em estudos futuros. Ademais, a terapia farmacológica só deve ser adotada quando tratamentos alternativos não se mostram efetivos.
