Abstract
En tant que « processus conférant aux populations les moyens d’assurer un plus grand contrôle sur leur propre santé, et d’améliorer celle-ci » (Charte d’Ottawa, 1986), la promotion de la santé suscite de nombreuses questions éthiques en lien avec la santé, sa définition et sa place dans la société, mais aussi quant aux notions de choix et de liberté, aux rapports de pouvoir et à l’importance res- pective des déterminants individuels et collectifs de la santé. Les réponses apportées à ces ques- tions essentielles ont un impact considérable sur le rapport que les acteurs de la promotion de la santé établissent avec les publics auxquels ils s’adressent. Cet article explicite donc certaines questions éthiques en promotion de la santé. Il s’agit, d’une part, de questions liées aux interventions qui concernent les comportements (la moralisation des conduites, la stigmatisation, l’instrumentation de la peur), d’autre part, de questions liées aux inter- ventions sociales et politiques (la transparence et l’honnêteté du débat public, l’association des personnes concernées, la responsabilité des décideurs). Ces explicitations nécessitent de définir le sens donné dans cet article aux termes « promotion de la santé » et « éthique » dans le champ de la santé publique. Elles s’appuient également sur des exemples précis visant à illustrer, en référence à certaines valeurs en promotion de la santé, les questions qui se posent et la faisabilité d’actions respectueuses de ces valeurs.
