Introduction: The Masquelet technique; a staged procedure with polymethylmethacrylate (PMMA) spacer and subsequent autologous bone graft (BG), is a viable treatment option for complex hand injuries with significant segmental bone loss. Iliac crest autografts are commonly used for BG, but associated complications suggest alternative sites for bone graft harvesting. Materials and Methods: Since using the distal radius for bone graft in the hand has the advantage of same location for surgical and postsurgical management, we discuss the technique and perform a retrospective review of ten patients undergoing staged Masquelet procedure for hand segmental bone loss. Results: Using the distal radius as a donor for Masquelet procedure in the hand is feasible and seems to have similar results and fewer complications as compared with other BG donor sites. Discussion: The distal radius as a bone graft donor has multiple advantages and similar outcomes compared with other donor sites for bone graft when using the Masquelet technique in the hand.
