FRAGILIDADE E CÂNCER NO IDOSO: DESAFIOS NA ESCOLHA TERAPÊUTICA

Abstract
O envelhecimento populacional representa um dos maiores desafios contemporâneos para os sistemas de saúde, especialmente diante da elevação da incidência de doenças crônicas e degenerativas, como o câncer. Entre os idosos, o processo de adoecimento oncológico é marcado por alterações fisiológicas, funcionais e emocionais que ampliam a vulnerabilidade clínica e comprometem a resposta terapêutica. Nesse contexto, a fragilidade surge como uma condição determinante, caracterizada pela redução das reservas fisiológicas e da capacidade adaptativa, influenciando diretamente a escolha do tratamento e os desfechos clínicos. O presente estudo teve como objetivo analisar os desafios enfrentados na escolha terapêutica de pacientes idosos com câncer em condição de fragilidade, considerando aspectos clínicos, funcionais, psicológicos e éticos. Foi realizada uma revisão bibliográfica integrativa nas bases Google Acadêmico, SciELO, PubMed e bibliotecas virtuais, abrangendo publicações entre 2016 e 2025. Os resultados evidenciaram que a prevalência de fragilidade é elevada entre pacientes idosos oncológicos e está associada à idade avançada, múltiplas comorbidades, baixa escolaridade e sedentarismo. Observou-se, ainda, que a avaliação geriátrica ampla, o uso da Escala de Fragilidade de Edmonton e a introdução precoce dos cuidados paliativos contribuem para um tratamento mais seguro, ético e humanizado. Conclui-se que a integração entre oncogeriatria, cuidados paliativos e abordagem biopsicossocial é essencial para a promoção da autonomia, da dignidade e da qualidade de vida do idoso com câncer.
