Incidencia de diabetes mellitus tipo 2 en pacientes síndrome post COVID-19. Una revisión sistemática y meta-análisis

Introducción: Investigaciones contemporáneas evidencian que individuos aquejados por el síndrome posviral asociado al SARS-CoV-2 (SPC) podrían manifestar desajustes homeostáticos, entre ellos la diabetes mellitus tipo 2 de aparición reciente. En virtud de ello, se propuso compendiar de manera rigurosa la información científica existente respecto a dicha conexión. Método: Se ejecutó una indagación sistemática acompañada de un meta-análisis, explorando repertorios bibliográficos como PUBMED, Scopus, Web of Science, EMBASE y LILACS, abarcando publicaciones comprendidas entre marzo de 2020 y el 17 de febrero de 2025, centradas en la emergencia de DM-2 en sujetos con SPC. Se incluyeron pesquisas de cohortes, tanto retrospectivas como prospectivas, que examinaron la frecuencia de diagnóstico reciente de DM-2 en personas mayores de edad con infección previa por COVID-19 confirmada, sin antecedentes de disglucemias o diabetes. El sesgo metodológico se valoró mediante la escala Newcastle-Ottawa, y se calcularon proporciones de incidencia con márgenes de confianza del 95%. La disparidad entre estudios se examinó con la prueba de Higgins-Thompson. Resultados: Se integraron cinco estudios en el análisis combinado, hallándose una razón de incidencia de 1.79 (IC 95%: 0.75-4.26, p=0.189), con un rango de HR/RR entre 0.90 y 8.94. La tasa acumulativa fue de 2.37%. Se observó una heterogeneidad pronunciada (I²=99.9%, p<0.001), denotando una dispersión significativa entre los datos. Análisis: Los hallazgos apuntan a un incremento del peligro de aparición de DM-2 en pacientes con SPC; sin embargo, la notoria heterogeneidad detectada obstaculiza la extrapolación concluyente. Se recomienda la realización de futuras investigaciones con protocolos metodológicos estandarizados para dilucidar esta asociación y fomentar lineamientos eficaces de vigilancia y profilaxis.
