Abstract
Mục tiêu: Xác định tỷ lệ, đặc điểm biến chứng thần kinh ngoại biên (TKNB) bằng thang điểm Toronto (TCNS) và phân tích mối liên quan giữa thời gian sử dụng Metformin với biến chứng này ở bệnh nhân đái tháo đường (ĐTĐ) típ 2. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang được thực hiện trên 400 bệnh nhân ĐTĐ típ 2 đang điều trị bằng Metformin tại Bệnh viện Đại học Y Thái Bình từ 06/2024 đến 05/2025. Biến chứng TKNB được đánh giá dựa trên thang điểm TCNS. Mối liên quan được phân tích bằng hồi quy logistic. Kết quả: Tỷ lệ bệnh nhân có biến chứng TKNB là 44,5%, trong đó mức độ nhẹ chiếm đa số (34,0%). Các triệu chứng cơ năng thường gặp nhất là cảm giác bất thường ở bàn tay (57,8%) và tê bì bàn chân (55,0%). Khám thực thể ghi nhận giảm phản xạ gót (66,3%) và giảm phản xạ gối (57,0%) là phổ biến nhất. Thời gian sử dụng Metformin trên 5 năm có mối liên quan với nguy cơ mắc TKNB cao hơn 2,5 lần so với nhóm dùng từ 1-5 năm (OR = 2,5; KTC 95%: 1,7-3,3). Kết luận: Gần một nửa số bệnh nhân ĐTĐ típ 2 dùng Metformin có biểu hiện TKNB. Thời gian sử dụng Metformin kéo dài là một yếu tố nguy cơ quan trọng liên quan đến biến chứng này.
