Blood glycated hemoglobin (Hb A1c) is a recommended biomarker for diagnosing and monitoring diabetes. Ensuring the stability of Hb A1c in stored blood is essential for accurate measurement. Although it has been shown that Hb A1c remains stable for at least 1 year at −80°C to −70°C (1), 2 weeks at 4°C (2), 1 month at −20°C (3), and 3 days at room temperature (15°C to 30°C) (4), the stability of Hb A1c during onboard storage on the analyzer has not been studied. In addition, the cumulative processing time of samples on the analyzer can result in delays. Hospital laboratories also frequently receive requests for additional testing on the same day for previously collected specimens. To accommodate these requests, laboratories routinely store specimens for a defined period (1 to 4 h) at RT after the initial testing before long-term storage. This practice ensures sample availability for add-on tests and prevents additional phlebotomy. Storing samples on an instrument system at RT and reusing them for add-on tests may present stability concerns and a risk of carryover. Therefore, this study investigates Hb A1c stability in whole blood stored on the analyzer and the potential carryover into adjacent blood and serum specimens.
