Introdução: A impactação dos caninos superiores permanentes é uma condição relativamente frequente na prática odontológica, com potencial de comprometer a morfologia e a vitalidade dos dentes adjacentes, especialmente os incisivos laterais superiores. Objetivo: Relatar um caso de reabsorção radicular dos incisivos laterais superiores associada à impactação bilateral dos caninos superiores permanentes. Relato de caso: Paciente V.M.M., 11 anos, sexo feminino, foi encaminhada ao ortodontista para instalação de aparelho ortodôntico. Durante a análise dos exames de imagem, observou-se reabsorção radicular dos elementos 12 e 22, secundária à impactação dos elementos 13 e 23. O exame clínico evidenciou ausência dos caninos superiores no arco dentário e espaço insuficiente para sua erupção. Optou-se pelo tratamento endodôntico dos incisivos acometidos, com vedamento dos condutos utilizando Biodentine (SEPTODONT). Atualmente, a paciente encontra-se em acompanhamento ortodôntico para tração dos caninos, sem sinais de alterações nos dentes tratados. Considerações finais: O caso relatado evidencia que a impactação de caninos superiores pode ocasionar complicações severas, como a reabsorção radicular de dentes adjacentes. O diagnóstico precoce, obtido por meio de exames de imagem, é fundamental para o planejamento terapêutico adequado. Ademais, o uso de biomateriais como o Biodentine mostrou-se relevante por oferecer excelente vedamento, elevada biocompatibilidade e capacidade de estimular o reparo tecidual, contribuindo para a previsibilidade e longevidade do tratamento endodôntico.
