Le ralentissement psychomoteur est un symptôme dont l’histoire se confond avec celle de la dépression. Il est décrit comme un symptôme central ainsi qu’un indicateur de la sévérité de la dépression au sein de la CIM 10 et du DSM V. Le diagnostic et l’optimisation de la prise en charge de la dépression sont des enjeux majeurs de santé publique. A travers une évaluation plus fine et ciblée, le ralentissement psychomoteur semble offrir des pistes d’amélioration de la sémiologie clinique, et être un marqueur annonciateur de réponse positive aux différentes thérapeutiques. Cette thèse soutient que le ralentissement psychomoteur dans la dépression est un solide indicateur pronostique et diagnostique. De plus, elle questionne la validité et la place des différentes mesures du ralentissement psychomoteur face à ces enjeux sémiologiques. Nous avons pu observer que la mesure du ralentissement psychomoteur, par l’Echelle de Ralentissement Dépressifs, associée à la mesure du statut cognitif par la Montreal Cognitive Assessment, permettait la distinction entre les patients bipolaires et les patients unipolaires en phase dépressive. Ces résultats confirment la valeur diagnostique du ralentissement psychomoteur dans la dépression. Nos résultats ont également montré que certaines évaluations comportementales du ralentissement psychomoteur (test de fluence verbales, Trail Making test A, test de force maximale en préhension) pouvaient s’avérer être des indicateurs d’une réponse positive à un traitement par rTMS des patients unipolaires. Cette thèse apporte des éléments théoriques et cliniques essentiels à travers l’évaluation des composantes motrice, cognitive et verbale du ralentissement psychomoteur.
